I hereby declare that I have read this Information Notice and that, within the scope of the Turkish Law No. 6698 on the Protection of Personal Data (“KVKK”), I give my explicit consent to the collection, recording, processing, storage and, where necessary, transfer of my personal data by Mak Medikal (referring to Dülger Grup A.Ş.) in accordance with the principles and procedures set forth under the applicable legislation.
I explicitly consent to the processing of all personal data, special categories of personal data and personal health data that I have provided or will provide to Mak Medikal verbally, in writing and/or electronically, including data that identifies or makes me identifiable, for the following purposes:
Conducting job application and candidate selection processes,
Establishing and performing employment contracts,
Providing and planning healthcare services,
Fulfilling legal and regulatory obligations,
Carrying out internal operational and administrative processes.
I further consent to the sharing of my personal data, special categories of personal data and health data, to the extent required by applicable legislation, with authorized public institutions, private entities and third parties for the purposes stated above.
Within the scope of KVKK, personal data refers to any information relating to an identified or identifiable natural person.
Special categories of personal data include data relating to a person’s race, ethnic origin, political opinions, philosophical beliefs, religion, sect or other beliefs, clothing and appearance, membership of associations, foundations or trade unions, health, sexual life, sexual orientation, criminal convictions and security measures, as well as biometric and genetic data.
I hereby give my explicit consent to the processing of the following personal data and special categories of personal data in accordance with KVKK and relevant legislation:
Identity Information: Name and surname, Turkish ID number, nationality, mother’s and father’s name, place and date of birth, gender; identity card, driver’s license and passport information, tax number, social security number, signature information, vehicle license plate details, etc.
Contact Information: Phone number, address, e-mail address, fax number, IP address.
Physical Location Security Data: CCTV recordings, fingerprint records, building entry and exit logs.
Visual and Audio Records: Photographs and video recordings (excluding records within the scope of physical premises security).
Health and Biometric Data: Health data including blood type, biometric data and other special categories of personal data defined under Article 6 of KVKK.
Request and Complaint Data: Personal data related to any requests, applications or complaints submitted to Mak Medikal.
